Injuries by Incident

This screen displays incidents for external and internal persons- by selected incident number. This window also allows the user to view, change, and add injuries by incident.Injuries for internal people (employees on your database) or external people (Job applications or non employees, i.e. contractors) can be recorded as well.

  • A Hazard/Incident has to be inserted before entering Injury details.
  • Nature of Injury
  • Bodily Location of Injury
  • Degree of Incapacity
  • Type of Treatment
  • Claim Status
  • Legally Reportable

Rules and Guidelines

  • A Hazard/Incident has to be inserted before entering Injury details.
  • Existing incidents can be maintained and changed in this option as well as entering new Incidents.

Field Information

ClosedID Type

This field displays the ID type of the person.

Option Description
Internal Employees on the database

 

ClosedID Number

This field displays the internal employee number (ID) of the person injured.

ClosedNature of Injury

This field displays the nature of the injury.

ClosedBodily Location

This field displays the location/area of the body that was injured in this Incident.

ClosedInjury Reported on Date

The date the injury was reported.

ClosedInjury Reported Time

This field displays the time the injury was reported.

ClosedReported To

This field displays the employee ID number of the person to whom the injury was reported.

Drill Down options

ClosedWitness

This window displays details of the person who witnessed the injury can be recorded.

  • This is a text field of up to 30 characters.

 

ClosedHours Worked Continuously

This field displays the number of hours worked on the day prior to the incident.

ClosedDegree of Incapacity

This field displays the degree of incapacity.

ClosedPercentage of Incapacity

This field displays the percentage of incapacity caused by the injury.

ClosedAttending Medico

This field displays the details of the attending medical person can be recorded.

  • This is a text field of up to 23 characters.

 

ClosedMedico Telephone Number (see above regarding Medico)

This field displays the telephone number of the attending medical person.

ClosedType of Treatment

Select the type of treatment required at the time of the injury.

ClosedClaim Status

This field displays the status of the claim in relation to the injury.

ClosedStatus Date

This field displays the date the claim status is changed

ClosedClaim Number

This field displays the claim number for the injury.

ClosedDate of First Symptom

This field displays the date the first symptom occurred for the injury.

ClosedLegally Reportable

Does this injury need to be reported to any legal authority?

ClosedFatality

Is this injury a fatality?

ClosedRehabilitation Program

Does this injury require the person to attend a rehabilitation program?
